APS staff can create online forms for your department or other needs. These forms can support legal electronic signatures and file uploads as well as a workflow of approvals.
Dynamic Forms is a cloud based form tool that will store the data recieved by the form for up to one year. Data can also be downloaded as a comma separated file (CSV) for use within Excel or other applications as well as email.
If you need a form, you can request one be created by APS staff.
